What is Haemorrhoids

Haemorrhoids are enlarged blood vessels around the anus (back passage). They can cause bleeding and itchiness, and sometimes are very painful. The word haemorrhoid comes from the Greek haim for blood and rhoos for stream (hence, bleeding). Haemor-rhoids are also known as piles.
The two types of haemorrhoids are: internal and external.

Internal haemorrhoids are inside the back passage where the rectum joins the anus. They are covered by the mucous lin-ing of the rectum. Internal haemorrhoids are enlargements of the normal cushions of blood vessels. They may be aggravated by constipation and straining.

External haemorrhoids are on the out-side of the back passage and are covered by skin. These are often enlarged due to stretching of the anal skin, as may occur during childbirth or straining.

CAUSES OF HAEMORRHOIDS

  • A Western diet that is typically high in processed food and low in fibre and bulk, resulting in constipation (hard stool)
  • straining to pass a motion due to con-stipation
  • pregnancy and childbirth
  • chronic coughing
  • vomiting
  • obesity
  • lifting heavy weights incorrectly
  • age.

MINIMAL INVASIVE SURGERY FOR HAEMORRHOIDS

HAL-RAR (Haemorrhoidal Artery Ligation and Recto-Anal Repair) is a minimally invasive surgical technique used to treat symptomatic hemorrhoids, especially Grades II to IV. It combines Doppler-guided ligation of hemorrhoidal arteries with a mucosal lifting procedure to address prolapse.

Procedure Overview

  • HAL (Haemorrhoidal Artery Ligation): A Doppler probe identifies arteries feeding the hemorrhoids. These are ligated to reduce blood flow, causing the hemorrhoids to shrink.
  • RAR (Recto-Anal Repair): For prolapsing hemorrhoids, a running suture lifts and secures the prolapsed tissue back into position.

Benefits

  • Minimal pain: Ligation occurs in the lower rectum, which has few pain receptors2
  • Quick recovery: Most patients resume normal activities within 24–48 hours
  • Low recurrence: Effective for both bleeding and prolapse symptoms
  • Outpatient-friendly: Often performed under mild sedation or spinal anaesthesia

Laser proctology

Laser technology is reshaping proctology by offering minimally invasive solutions for common anorectal conditions like hemorrhoids, anal fissures, fistulas, and pilonidal sinus. Here’s a quick overview of its clinical applications and advantages

Common Laser Procedures in Proctology

  • Laser Hemorrhoidoplasty (LHP): Shrinks hemorrhoidal tissue with minimal bleeding and pain.
  • Fistula Laser Closure (FiLaC): Targets fistulous tracts while preserving sphincter function.
  • Laser treatment for Anal Fissures: Promotes healing and reduces sphincter spasm.
  • VALAPS (Video-Assisted Laser Ablation of Pilonidal Sinus): Minimizes excision and improves recovery.
  • Laser excision of anal warts: Offers precision and reduces recurrence.

Advantages Over Conventional Surgery

  • Reduced postoperative pain and faster recovery
  • Minimal bleeding due to superior coagulation
  • Lower risk of infection and scarring
  • Preservation of continence in fistula cases
  • Shorter hospital stay and quicker return to normal activities
